I got the pattern from Helen Shaw’s Flies for Fish and Fisherman. It is, apparently, based on the Polka Dot Wasp Moth or Oleander Moth. I could not find out any information about the fly. I would it originates in an area that has a lot of oleander bushes and Oleander Moths but that is just a guess.
