Sure, there are exceptions.  Likewise, some CPU traps do affect the
CPU state: for instance a memory fault trap can update the Page Table Entry
in the MMU as a side effect of being run, or even cause a whole page to be
duplicated in copy-on-write implementations.
However, people writing special-purpose extensions like the one you mention
must be extra careful to not induce harmful side effects to the code that
use them.
This is very different from the average utility module on CPAN whose fairly
innocent-looking DESTROY can modify the behaviour of my (otherwise correct)
program.
