WordPress has a native function since version 2.8.0 which is filterable for generating the lostpassword URL, you should definitely use it instead of hardcoding it.

Which is filterable with: https://developer.wordpress.org/reference/hooks/lostpassword_url/In your code there are 2 references of hardcoded lostpassword url generation:
