minor documentation fix

* shoes/app.c: no fill on the default link hover. an ugly thing.

* shoes/ruby.c: Text#cursor_left and Text#cursor_top. should have a way to get coordinates of any character or line as well.

* shoes/ruby.c: new :wrap style. "char" to allow lines to wrap between characters or "trim" to cut lines and end with an ellipsis.

* Rakefile: package the libportaudio dynamic lib with shoes on osx.

* req/bloopsaphone: include bloopsaphone, add as part of the build process.

* shoes/native/cocoa.m: new `ask` dialog on osx that works on tiger. also could be basis for sheet-style dialogs.

* lib/shoes.rb: return anonymous binding under Ruby 1.8, still need to figure out an equivalent for 1.9.1.

* shoes/native/windows.c: more progress toward layering and flickr-free nesting among complex windowing situations. still need some z-ordering code (across all platforms) to layer text and graphics above nesting windows, if they have the higher z-order.

* shoes/native/windows.c: sorted out some win32 painting, layering of nested windows. needed WS_CLIPSIBLINGS to mask some layers and was able to limit RedrawWindow to only repaint changed surfaces.

* bugs: two bugs found by kyle king.

* static/manual.txt: note about Kernel.exit, recommended by louis-phillipe.

* bugs: the "verify archive" message, found by jan martin.

Fix class hierarchy in manual by ignoring mixins in introspection

* req/json/xt/generator/generator.c: RFLOAT_VALUE undefined in Ruby 1.8.x.

* Rakefile: under ruby 1.9.1 on linux, copy the shared lib to libruby.so.1.9.

* req: fixing extensions to work under both ruby 1.8 and 1.9. mostly header files and struct layouts at play here.

* shoes: first attempt to bring shoes up to ruby 1.9.1-rc2. still need to sort out some incompatibilities with 1.8.
