Describe the bug
When trying to attach a volume, either by using the UI or kubectl commands, it remains stuck in an attach/detach loop.
To Reproduce
Steps to reproduce the behavior:
- Go to Longhorn UI > Volume.
- Click on 'Create Volume'. Use a random name, size 1Gi, everything else left on default. Click 'OK'.
- Try to attach the newly created volume to any node.
- See error: volume stuck in attaching/detaching loop.
Expected behavior
The volume should be successfully attached to the desired node.

Environment:
- Longhorn version: v1.1.0
- Kubernetes version: v1.20.2 (1 master node, 2 workers)
- Node config
- OS type and version: CentOS 8
- CPU per node: 8
- Memory per node: 16Gi
- Underlying Infrastructure: VMWare
- Number of Longhorn volumes in the cluster: just the one trying to attach (fresh install)
Additional context
- Cluster is IPv6 only.
- Attaching a volume with ISCSI frontend mode works.
- On each node,
/var/lib/longhornis using a separate 16Gi disk (i.e.mount /dev/sdb /var/lib/longhorn) - Environment check script was successful.
- Open-iSCSI is running on all the nodes. Here are the logs from the node to which I was trying to attach the volume:
